Computed tomography for the diagnosis and management of idiopathic cervical spine hyperostosis

Abstract:
Swallowing disorders due to diffuse idiopathic skeletal hyperostosis (diSh) often remain undetected, even if symptoms increase over the years. We present the case of a 72-year-old woman with an eight-year swallowing disorder history. a cervical spine ct highlighted the hyperostosis with a consequent anterior displacement of oropharynx. authors used the Mader Classification in the DISH evaluation, according to a multidisciplinary approach of the disease. The patient suffering from hypertension, but not from diabetes mellitus and presented a BMi of 23 kg/m2. a conservative approach was used; nSaids contributed to effective analgesia and the treatment was well accepted from the patient, also during the investigation of a comfortable position; however, these drugs are not able to improve the degree of movements. a longterm treatment is difficult and during the disease progression, loss of weight, dyspnea, dysphonia and/or food aspiration may occur.
